Dimensions Memory Care was initially funded by a grant from the Minnesota Department of Health and created by Marysue Moses, a renowned expert in memory care.
The Dimensions program stresses the importance of understanding the needs of a person with dementia. Designed to ensure that memory care residents receive exceptional care, Dimensions memory care relies on evidence-based practices.
We provided personalized care plans based on detailed assessments that consider the person’s abilities, needs, preferences, and personal history. Additionally, we provide activity boxes filled with meaningful tasks tailored to residents' cognitive and physical abilities, focusing on different stages of dementia. Caregivers help ensure residents’ comfort and well-being by focusing on hydration, nutrition, activity and rest.
In addition to comprehensive aging and dementia care training for all staff at New Employee Orientation, the “Champion Initiative” trains Memory Care Champions to disseminate training and build a team of skilled and empathetic advocates for residents living with dementia. We regularly provide hands-on education and training for front-line staff focused on specific dementia-related topics. “Hot-Topic” training provides monthly skill-building opportunities for staff at every site. Dimensions Coordinators are available for consultation and training.
Our memory care program offers a variety of engaging activities to keep residents active and stimulated. Chair exercises with light weights are held on weekday mornings, while sit and stretch exercises are available on weekends. Residents love participating in fun games like balloon volleyball and Velcro ball, as well as dance parties. A highlight for many is the weekly Timeslips Storytelling session, where residents craft imaginative stories based on a picture prompt, promoting creativity and reminiscing. Creative arts sessions are also offered weekly, featuring music curated by a longtime resident DJ, who plays various genres and decades, creating a unique and enjoyable experience for all.
Our memory care neighborhood features dementia friendly design structures and amenities. Special lighting, designed for aging eyes, along with painting and decorating in calming colors ensure that residents feel comfortable and secure. With controlled access entries, community activity areas and outdoor patios, our memory care spaces provide a comfortable homelike environment.
As your partner in dementia care, the Dimensions Program offers support groups onsite. We also offer family support through education, care conferences, and community resources—including partnerships with AccentCare Fairview and M Health Fairview.
